There are some annoying plastic seals to hold the sump (the thing that holds the basket against the pressure head) that can get broken/lost. If that happens, water will escape around the sump instead of being forced through the beans. Unfortunately that piece is not usually available. I have epoxied a plastic shim to the sump key itself, but need a better long-term solution.
